  1. Obtain Form 6: You can obtain Form 6 from the local Electoral Registration Office, Voter Facilitation Center, or download it from the NVSP or the Election Commission of India’s website.
  2. Fill the Form Manually: Complete the form by providing all necessary details as required in the form.
  3. Attach Required Documents: Attach photocopies of your identity proof, address proof, and age proof along with the form.
  4. Submit the Form: Submit the completed form along with the attachments to the nearest Electoral Registration Office or hand it over to the Booth Level Officer (BLO) in your area.
